Setting up an LLC (NJ)

Hello All,
Me and a friend want to start a restaurant business. We plan to divide everything equally 50/50.
How should we create this LLC? When reviewing this page https://www.njportal.com/dor/businessformation/home/welcome
It only asks for one owners name and address and ein/SSN then they can add a manager. Does it matter who’s name, address and EIN is on it initially ?
