What tools do you use to create and track SOP's & Workflows?

Looking for some suggestions if possible. We've built up a tangled horrible mass of tools and systems across a number of businesses and trying to find a way to centralise ops and document everything is becoming a real nightmare. Wondered if anyone here has managed to get their SOP's down to a fine art?
