Creating a price calculator

So much to my relief.. I finally figured out an equation to set my rates for each of my new customers. I was wondering what's the best way or in what do you any of you, who use a form of calculator do so?

Ultimately I would like to create a kind of form for the calculator. at least in the sense of UI.

I wanted something I could both access from my phone and PC. I considered Excel, though I've used it in the past, my experience with the app is anything but what one would call frequent. So I'm a little rusty. but any other apps? again just kind of want to make a form to calculate rates.
