What employee survey questions do you use?

Wondering what questions you ask internally and how they've been useful. Do you have feedback sessions or something else?

I've heard some references to Laszlo Bock but Google is way too big of a company to model on. What has worked for you?

What do you do to ensure the employees stay happy and tell you what's going wrong? There's the option to take a dedicated HR specialist but it feels like abandoning.
