Is there a more Professional, Free Alternative for Google Forms?

I have an e-mail list and want to occasionally ask questions, do polls, etc... But Google Forms looks really unprofessional.

Is there something like Google Forms, that is simple, not a lot of complicated features, just so people can answer questions and send files?
