How do you handle performance reviews and feedback?

Hey guys, how do you handle performance reviews and giving feedback to employees? Do you use lots of objective data or (lets be honest) often rely on how it feels the person is doing? Also do you do it at regular intervals or just when someone requests feedback? What’s your process?

Would love to see what works best for you guys and what issues are you facing there!
